Annual Performance Reviews (APRs) serve as an important tool for discussing faculty members' performance, goals, and progress toward promotion, and toward tenure for tenure-eligible faculty.
Policies & Procedures
University Handbook for Appointed Personnel (UHAP 3.2) outlines the policies and procedures for the Annual Performance Review for the following faculty:
- Tenured faculty and tenure-eligible faculty
- Continuing-eligible faculty and continuing-status faculty
- Career-track faculty (including all lecturer, instructor, and professorial titles with this designation)

Faculty Profile
Faculty Portfolio, is a centralized activity tracking tool for Annual Profile (annual performance reviews [APR]), UA Profiles, and Review, Promotion, and Tenure (RPT). This Annual Profile system allows faculty members to enter their contributions in teaching, research, and service/outreach throughout the year in preparation for their APR for the Calendar Year APR cycle. The system also feeds each user's information to UA Profiles so that faculty members' achievements are accessible on a public website.

Workload Distribution
Faculty members must enter their workload percentages for the calendar year in Annual Profile in the “Workload Distribution” field.
Curriculum Vitae (CV)
Each faculty member must upload a CV (Word or PDF format) to the system. The University suggests dossier template Section 4 for a CV format.
Annual Review Narrative
Each faculty member must upload a narrative (Word or PDF) to the system.
In general, this narrative should provide a summary of the individual’s performance in the categories of teaching, research and service/outreach (where applicable) during the past calendar year. However, it is essential that faculty members refer to their departmental policies for specific guidance on what should be included and any page/word limits.
List of Courses and Student Course Surveys (SCSs) in all classes for the prior calendar year.
Course lists are auto populated from UAccess. Each faculty member should verify their course list in the system prior to Reading Day each semester. If there are errors, please contact the departmental course scheduling coordinator to have the information corrected with Room & Course Scheduling.
Student Course Surveys (SCSs) will be auto populated, where applicable. Course evaluations that were not performed/ordered (e.g., small class size, independent study) are excluded.
Faculty Workload Agreements
Work with the department head to complete your Faculty Workload Agreement Form for the upcoming Academic Year.
Departmental Peer Review Committee’s Review /Recommendations
The committee, guided by department and COH guidelines, must provide the following, using the Annual Profile’s evaluation forms/fields:
Ratings:
- Meets or exceeds expectations
- Does not meet expectations
Committees can make further recommendations about the additional ratings that are available to the head (see row below), using the comments field in Faculty Portfolio.
Comments:
The committee must provide comments describing the rationale and results of the peer review. Committees are encouraged to follow these guidelines so that faculty receive more formative feedback for their annual review. This feedback will be shared with the head and the reviewee.
Supplemental documents (e.g., letters) may be uploaded, but the required rating and comments fields must still be entered in the system.
Department Head /Director /Supervisor Evaluation
Guided by department and COH guidelines, the head must provide the following, using the Annual Profile evaluation forms/fields:
Ratings:
- Truly exceptional
- Meets or exceeds expectations
- Does not meet expectations – Needs improvement
- Does not meet expectations – Unsatisfactory
Comments:
For purposes of formative feedback, the head, director, or supervisor must provide comments describing the rationale and results of their review. If a faculty member is tenure-eligible, heads should provide feedback about their progress toward promotion and tenure. If a head disagrees with the departmental peer review committee’s ratings, a rationale must be provided. Comments can also include a discussion about goals, assignments, and expectations for the upcoming year.
Supplemental documents (e.g., letters) may be uploaded, but the required rating and comments fields must still be entered in the system.
Faculty Workload Agreements
The department head should discuss and complete the Faculty Workload Agreement for the upcoming academic year.

Workload agreement forms for the upcoming Academic Year will be completed before the end of the annual review process. Completed and signed copies of the Workload Agreement forms must be given to the dept. or school Faculty Affairs Coordinators for input into UAccess Employee Workload modules.
